Grace Health Earns Prestigious 2024 CHQR Badges for Quality Improvement

Corbin, KY, September 17, 2024 – Grace Health is proud to announce the achievement of several distinguished Community Health Quality Recognition (CHQR) badges for 2024, acknowledging its commitment to quality care, health equity, and the enhancement of patient access and health information technology (HIT). This year, they earned the National Quality Leader (NQL), Access Enhancer, Health Disparities Reducer, and Advancing Health Information Technology (HIT) for Quality badges. 
CHQR badges recognize health centers with significant improvements in clinical quality, patient access, health equity, and health information technology. Eligibility for these badges requires health centers to be active participants in the Health Center Program, submit complete data on time, and report on clinical quality measures (CQMs) using an electronic health record system.
The NQL badge is awarded to health centers performing in the top 11-20% for CQM. Grace Health received silver status, an improvement from bronze last year, which is a testament to its continued focus on delivering top-tier care. Grace Health met or exceeded national benchmarks for vital CQM groups to achieve silver status, including behavioral health, cancer screening, diabetes health, heart health, HIV prevention and care, and maternal and child health.
The Access Enhancer badge recognizes health centers that have increased patient access by at least 5% in total patients or specific services, such as mental health, substance use disorder, dental, or enabling services. Grace Health made significant strides in expanding access to critical services, furthering its commitment to community health.
The Health Disparities Reducer badge reflects Grace Health’s success in reducing health disparities by improving clinical outcomes in low birth weight, hypertension control, and uncontrolled diabetes for underserved groups. By performing well in these areas and qualifying for the Access Enhancer badge, Grace Health continues to make meaningful progress in health equity.
Grace Health demonstrated excellence in utilizing health information technology to improve care delivery, earning it the HIT for Quality badge. This badge honors Grace Health’s adoption of an electronic health record (EHR) system, offering telehealth services, exchanging clinical data with key providers, and collecting patient social risk factors data, all vital components of modern healthcare.
